The Middletown Works Hourly and Salaried Union Retirees Health Care Fund (Middletown
Works Retirees Fund or Plan, for short) provides medical, prescription drug, dental, vision,
and life insurance benefits. This Summary Plan Description (SPD), which also serves as
the Plan Document, describes the benefits available to eligible retired employees and
Dependents as of January 1, 2025.

- If you receive an explanation of benefits (EOB) showing a payment to the provider for a claim that you have already made payment to the provider for, please reach out to the provider directly for a refund.
